Your School Readiness Health Questionnaire

If your child is starting at a Warwickshire school in September, we’re asking you to complete an online health questionnaire called the School Readiness Health Questionnaire. The reason for this is to identify any health and wellbeing needs your child may have and ensure they get the right support so that they can reach their full potential as they start school.

Who are Connect for Health?

We’re the local school nursing service for Warwickshire. We take over from Health Visiting when your child starts school and support them up to age 19, or age 25 for children with SEND. You can contact our service at any time if you have concerns about your child’s health and wellbeing. The service is commissioned by Public Health Warwickshire and provided by registered charity Compass. Learn more about Connect for Health on our service webpage.


What will you do with the information I provide about my child?

The questionnaire is hosted securely online by our partner School Screener. The information you provide is treated confidentially. Your answers will trigger on screen advice and support messages where appropriate and may be reviewed by one of our nurses, who could contact you to offer further health advice, information and support. With your consent, the nurse may also liaise with school if support is required within your child’s school day. You will also be able to download a copy of your completed questionnaire including the on screen messages – you will receive instructions on how to do so via email.

The data from all questionnaires completed is anonymised and compiled into school reports, which help school staff understand the health and wellbeing needs of their new intake of pupils. The anonymised data is also used to provide a local and regional overview of health needs, which helps us to tailor our service and ensure families are offered the right support.

Connect for Health follow Warwickshire Safeguarding Children’s Board policies and procedures. If the answers to your questionnaire suggest your child is at risk of harm, we may share this information with other organisations if necessary to keep your child safe.

My child is perfectly healthy – do I still need to complete the questionnaire?

Even if you feel your child doesn’t have any unmet health and wellbeing needs, we still strongly recommend you complete the questionnaire. It’s a great opportunity to stop and review your child’s health and development before they start school, and you’ll receive useful advice and websites along the way. In addition to this, the anonymised data from your questionnaire will help schools and other families by contributing to an overview of reception-aged children’s health needs locally.
